The HIPAA (Health Insurance Portability Accountability Act) privacy rule gives individuals the right to adequate notice of the uses and disclosures of protected health information that may be made by Quality Home Health, Inc. and of the individual's rights and the agency's legal duties with respect to the protected health information.

The Notice of Privacy Practices describes the practice of Quality Home Health, Inc. and describes how we may use and disclose information about you.

Your individual rights fall into four areas under HIPAA:

  1. The right to request privacy protection for protected health information
  2. A right of access by individuals to protected health information
  3. A right to amend protected health information
  4. A right to an accounting of disclosures of protected health information

Quality Home Health, Inc. may use or disclose protected health information without the individual's written consent for the following purposes:

  • For treatment
  • For payment
  • For health care operations
  • For health related benefits and service
  • For judicial or administrative proceedings
  • For disaster relief/research/organ, eye, and tissue donations
  • For information on how we will contact you
  • To individuals involved in your care
  • For funeral directors/Coroners
  • Victims of Abuse, Neglect or Domestic Violence
